An opportunity has arisen for a Rehabilitation Officer to join our Team at Great Western Court.

You will be required to work 21 hrs per week on a rota basis including Bank Holidays and Weekends  

You must be able to write care/goal plans and administer personal care as required. An NVQ 3 in Health and Social Care is desirable but not essential.

You will also be required to administer medication.  

If you have the skills and experience to work within an integrated team providing support to adult service users in a bed based reablement and assessment unit, helping them to regain their independence and improve their quality of life then we would like to hear from you.

As a key member of the integrated team you will be required to support the delivery of the reablement pathway, identifying and working towards outcome focused goals, functional and social, based upon initial assessment and regular review.
