The Cotswold Locality Team is located in the heart of the Cotswold’s, with office bases in Cirencester and Bourton-on-the Water. It is part of the established and experienced Adult Social Care team who work alongside health professionals and local community partnerships to provide the best possible outcomes for service users.

We are looking for a Social Care Assessor to join this team and help make the difference. The right person for the role will be focused on maximising independence and be able to undertake a range of fieldwork support duties in order to enable and assist people in the community to access appropriate services.

The work will be challenging, with a constantly changing caseload. You will be involved in assessing the needs of older people and adults who have a physical disability or a dementia type illness and their carers within the community.

You will need to be reliable, skilled at working as part of a team, flexible, have good communication and IT skills, and be able to work from other locality bases if required.
