Social Worker - Continuing Healthcare

Gloucestershire County Council has a vacancy for a Registered Social Worker with experience of working with the continuing healthcare framework. The role will involve Working within Gloucestershire’s safeguarding adult protection procedures and contributing to the continuing health care assessment process i.e. decision support tool (DST) and recommendation of eligibility where there is evidence of a primary health care need.
The general expectation is that a proportional care act assessment will be carried out to inform the DST. The assessment will provide good evidence based information about the care and support needs of the adult with care and support needs. Where the assessment indicates the likelihood of a primary health care need a rationale in support of this outcome will be inclusive in the assessment and lead to completion of the continuing health care checklist.
The role will involve supporting the development partnership CHC practice by the sharing of CHC information with locality champions for cascading to locality staff under the direction of the CHC operational lead.
