You will be required to work on an as and when needed basis and be part of our pool of relief carers.
You must be able to write and review care plans and be responsible with the help of the team for up to six service users.
If you have the skills and experience to work within an integrated team providing support to adult service users in a bed based reablement unit, helping them to regain their independence and improve their quality of life then we would like to hear from you.
As a key member of the integrated team you will be required to support the delivery of the reablement pathway, identifying and working towards outcome focused goals, functional and social, based upon initial assessment and regular review.
