The Older People Integrated Commissioning Hub has a vacancy for a Transformation Manager.


The successful applicant will lead on enhancing digital transformation amongst care providers. The work will include:



  • The development of a Care Home Digital Strategy and action plan to support care homes embrace digital opportunities.

  • Leading a stakeholder group to enact this work.

  • Identifying and managing resources to effect key actions from the strategy.

  • Laying the groundwork to extend the strategy with community providers.


The Adult Transformation Team is also looking for a Transformation Manager whose work will include:



  • Developing and leading ambitious projects and programmes

  • Acting as a positive advocate for change with a diverse group of stakeholders

  • Using experience of health and social care to deliver areas of work such as system redesign, remodelling of services and behaviour change


You will be able to work flexibly between two office bases and from home.


This is an excellent opportunity to work in an establishing Integrated Care System which ensures that health and care are considered as a joint provision across the county.
